Winners
will be sent an email notification, and will also be announced via the AXA
Philippines and Fail Proof Your Future Facebook pages. Prizes not claimed
within 60 days after notification will be forfeited.
10. Employees, sales distributors, and management of
AXA Philippines, Havas Media Ortega, Ogilvy Public Relations, and One Incentive
Systems Advocates (1ISA) are disqualified from joining the promotion.
Winners
Guidelines:
1.
Winners
will receive an email and SMS notification from AXA Philippines.
2.
To
claim their prize, winners must bring a printed copy of the email and two valid
government-issued identification cards (IDs) such as Philippine passport, SSS
ID, driver's license, postal ID, and Voter's ID. Prizes may be claimed during
office hours (9am to 5pm) on weekdays from One Incentive Systems Advocates,
Inc. (1ISA), at Suite 11-A Strata 100, F. Ortigas Jr. Road (formerly Emerald
Avenue), Ortigas Center, Pasig City.
3.
Representatives
of winners must bring a signed letter of authorization from the winner, two
valid government-issued IDs of the winner and two personal valid
government-issued IDs to claim the prize in behalf of the winner.
4.
Winners
of National Bookstore gift certificates, Gymboree memberships with gymplay
coupons, Nido Discovery Center and Inventors' Lab tickets, Mind Museum and
Manila Ocean Park passes and Yamaha School of Music and Center for Pop lessons
will be subject to the terms and conditions of each respective partner
merchant.
5.
Winners
of the Grand Prize of 3 days/2nights trip for four, 2 adults and 2 children
(aged 2-11 years old), to Legoland Malaysia must all have valid passports. The
Legoland Malaysia tour is inclusive of roundtrip airline tickets
(Manila-Singapore-Manila) for 2 adults and 2 children, roundtrip airport transfers,
overnight stay for 2 adults and 2 children in a hotel in Singapore on the first
night, transportation from Singapore to Legoland Malaysia on the second day,
overnight stay in Johor Bahru on the second night, transportation back to
Singapore on the third day and to airport for return flight to Philippines.
Adults are expected to ensure the care and safety of the accompanied children
throughout the trip. Travelers are expected to adhere to tour schedule and
guidelines. AXA Philippines will not be liable should travelers miss their
scheduled flight/transfers.
6.
All
prizes are transferrable but not convertible to cash.
7.
Prizes
not claimed within 60 days after receipt of winner's notification will be
forfeited.
